Annual Vehicle Inspection Form and Label

Annual vehicle inspection forms and decals are required by the FMCSA and DOT. Each vehicle must be inspected by a DOT approved inspection facility annually. The Annual Inspection Forms and Labels are proof that the vehicle has passed inspection in accordance with Code 396.17.
